Case from Dr Sadia Ammad
25 year old patient, used potent steroids on the face for the purpose of skin whitening for 8 months. A very common practice in the Middle East. Numerous microcomedones and increase in facial hair is visible.Comedones were manually removed and pateint was prescribed adapalene gel.

Steroid induced acne is usually monomorphic. I had previously seen it with oral steroids rather than topical. In my part of the world topical steroids on the face give rise to rosacea which does not have commedones which seems unusual!

I also agree with Dr.Ian. These are looking more like Gr.1 to2 Acne Vulgaris. In steroid induced acne ,even to potent local steroid, monomorphic papules are seen rather than open commedones. On face telangiectasia and rosacea are common.

I agree with both of you. The inflammatory monomorphic lesions of steroid acne are easily distinguished from the comedonal acne. So, I would label this image as a case of comedonal acne. May be she already had acne which was made worse by the use of topical steroids. Steroid induced roscea and telangiectasia are also commonly seen in my practice.

Drug induced acne is monomorphous without comedones. It is unlikely steroid induced
