25 year old patient, used potent steroids on the face for the purpose of skin whitening for 8 months. A very common practice in the Middle East. Numerous microcomedones and increase in facial hair is visible.Comedones were manually removed and pateint was prescribed adapalene gel.

Eczema craquelé or asteatotic dermatitis is characterized by pruritic, dry, cracked, and polygonally fissured skin with irregular scaling. The morphologic appearance of eczema craquele has been referred to as cracked porcelain and a dried-up riverbed. It most commonly occurs on the shins of elderly patients in winter months.

This farmer injured his hand on some wire. He later developed a nodule at the site of the injury and subsequently a discharging plaque. Other nodules then arose along the lymphatics up to the elbow. Bactrial cultures were negative. Tissue biopsy showed subcutaneous suppurative granulomas. No organisms were seen on special stains but culture grew Sporothrix schenckii.

This elderly lady was on holiday in Bali when she was injured by a motorcycle. She was put on some antibiotics and given anti inflammatories. Shortly after returning to Australia she developed these blisters on her feet and slightly on the first webspaces of her hands. Her skin tingled in the sun. Clinically this is pseudoporphyria due to her anti inflammatory Brufen.This condition typically has acral blisters but may lack the increased facial hair and skin fragility seen in true porphyria cutanea tarda. Patients also have normal porphyrin studies. Pseudoporphyria can also be seen with uremia and hemodialysis.

Nevus spilus or speckled lentiginous nevus is relatively common. Up to 2% of the population may have one. Some are present at birth but most appear in the first year of life. I have never seen one which changed but I have never specifically followed one up over a period of time either.

These were the fingers of an elderly, bedridden patient in a nursing home, where there was an unexplained outbreak of an itchy rash among the patients and staff. This lady had been thought to have keratoderma due to psoriasis but she had typical scabies elsewhere and examination of scales from her fingers showed scabies mites. This is crusted or Norweigian scabies.

Eruptive xanthomas appear in crops on the buttocks, extensor surfaces of the extremities, and flexural creases. Adequate treatment involves controlling the underlying hyperlipidemia with strict dietary therapy. Weight reduction and carbohydrate intake restriction are helpful in cases associated with insulin resistance.

This patient came with c/o extremely pruritic evanescent lesions on trunk and shoulders for the last 2 years. They used to appear during exercise and after taking a hot bath. O/E They were 3-5 mm edematous red urticarial papules. A few of them coalesced to form big plaques. They used to subside after taking a rest. This photograph was taken after giving the patient a brief exercise regime. He was prescribed Tab.Hydroxyzine 25 mg at bed time and advised to avoid vigorous exercise and hot baths.
