Case from Dr Ian McColl
This farmer injured his hand on some wire. He later developed a nodule at the site of the injury and subsequently a discharging plaque. Other nodules then arose along the lymphatics up to the elbow. Bactrial cultures were negative. Tissue biopsy showed subcutaneous suppurative granulomas. No organisms were seen on special stains but culture grew Sporothrix schenckii.

This man was treated with Itraconazole but what other treatments would be useful?

We used to use potassium iodide either in tablet form or as a saturated solution starting at three drops tds and increasing to 10 drops tds. It has a funny metallic taste and is best given in orange juice.
